Requirements
The treatment must be necessary to improve a deformity directly related to a congenital abnormality, a personal injury resulting from an accident or trauma, or a disfiguring disease.
A Letter of Medical Necessity from a licensed medical professional may be required to substantiate that the procedure is not solely for cosmetic purposes but is medically necessary to treat a qualifying condition.
